Electronic Periodic Health Assessment Login

Access PHA log in page:  https://eha.health.mil/EHA/

Logging on to the Electronic Periodic Health Assessment (ePHA) will require a CAC or registration and password.

Completing the PHA requires you to fill out approximately 25 pages of health information and takes 10-20 minutes. The value of this new Electronic PHA System over paper records is that once you complete the first time your answers are saved.

  • I cannot log in to ePHA system and it cant even get through the website.. Using MS edge, Chrome and Opera browser and all is not working. I just keep receiving this on all browser:

    Your connection is not private
    Attackers might be trying to steal your information from eha.health.mil (for example, passwords, messages, or credit cards).

  • Unable to log on to the eha.health.mil website to fill out my part 1 of the PHA. Below alert is received in Microsoft Edge, a similar response is received for Chrome and Firefox:

    eha.health.mil uses encryption to protect your information. When Microsoft Edge tried to connect to eha.health.mil this time, the website sent back unusual and incorrect credentials. This may happen when an attacker is trying to pretend to be eha.health.mil, or a Wi-Fi sign-in screen has interrupted the connection. Your information is still secure because Microsoft Edge stopped the connection before any data was exchanged.

    You can’t visit eha.health.mil right now because the website uses HSTS. Network errors and attacks are usually temporary, so this page will probably work later.

  • THIS IS THE SOLUTION TO YOUR PROBLEM:
    1. Use CHROME or EDGE ONLY!!
    2. Go to https://eha.health.mil/eha
    3. When the site says there is an error or privacy issue like CAPT Keniston shared, click in the middle of the page (trust me, I know it sounds crazy but just do it) and type this exact phrase, with no spaces “thisisunsafe” then HIT ENTER.
    4. You should be prompted to log in with your CAC pin.
